University Staff Benefit Statement
What is a University Staff Benefits Statement?
A University Staff Benefits Statement is provided to you if you are covered by the Retirement System, and it outlines the contributions that you and the university have made to the various benefit programs in which you are enrolled.
Statements reflect projected 2005 calendar year premiums and contributions for the insurance programs including retirement, Social Security and tax-sheltered annuity programs.
Insurance premium projections are based on April 2005 amounts. If you do not participate in a benefits program or did not have a deduction taken for an insurance program in April, the word “none” will appear in the coverage column.
Who gets a benefits statement?
Statements are sent to active, permanent university employees covered under the Wisconsin Retirement System (WRS). Limited-term employees covered by WRS do receive a statement, although it does not provide retirement information.
When do I receive my University Staff Benefits Statement?
A University Staff Benefits Statement should reach your campus mailbox during the third week of April.
What information is not in the statement?
There is no information about employee reimbursement account contributions or pre-tax transportation program contributions.
Why should I read my statement?
You should review your statement thoroughly for accuracy of the information. Immediately notify the person or office shown on your statement if you find any incorrect information.
What health coverage/disability insurance information should I review on my statement?
Do you have family health coverage but have no spouse or dependents? Both you and the university may be overpaying your premium.
Did you think you were carrying health insurance, but the statement indicates “none”?
Do you have family Excess Medical and Dental (EPIC) coverage, but have a spouse and no dependent children?
Do you have two-person excess medical and dental coverage and only need single?
n Do you think you have Income Continuation Insurance (ICI) coverage, but the statement indicates “none”?
Are you listed as unclassified faculty or academic staff, and is your ICI waiting period correct?
Dental insurance is represented or non-represented, single or family coverage based on your position title. What is listed for you?
What life insurance information should I review on my statement?
State Group Life Insurance: What life insurance survivor benefits are listed? Do you have the coverage level you selected for you, your spouse and/or your dependents? Are you carrying spouse and dependent coverage, but you have neither a spouse nor dependent? Do you think you had spouse and dependent coverage but have none? Are your dependents still eligible?
Individual and Family Group Life coverage for your spouse and dependents are independent of each other. Do you have both or neither? Is this correct?
University Insurance Association is employee-only coverage and is mandatory for all eligible faculty and academic staff, and is not available to classified employees.
UW Employees Inc. is employee-only coverage; benefits and premiums are determined by age.
Accidental death and dismemberment may be single or family.
